CREDIT CARD DONATIONS
If you desire to arrange for a contributor to pay by credit card, please print off the Credit Card Authorization Form, have the contributor complete it, and mail it to us along with the contributor's completed GOAL Contribution Form and page 2 of Form IT-QEE-TP1 signed by the contributor at part C to:

Upon receipt of the DOR Approval Letter and completed Credit Card Authorization Form as authorized by the contributor, GOAL will debit the contributor's card in the amount of the authorized payment. (The amount contributed may not exceed the amount of the contribution for which the contributor has been approved.)
In order for a taxpayer to receive the Qualified Education Expense Credit for 2011, payment must be sent to GOAL by 12-31-2011.
