Step 1: Application Form
Complete and return the
application form, and non-refundable application fee of $90.00 per student.
Step 2: Teacher Recommendation/Transcript Release Form
This release must be completed and signed by the applicant’s parent(s) and taken
to the school the applicant now attends. The applicant’s records will then be released
to Providence Christian Academy. Please encourage the school to expedite these records
to us as soon as possible to help complete your file. You may download and print
the
Release of Educational Records and
Teacher Recommendation Form for your convenience.
Step 3: Certificate of Immunization Form and Vision, Hearing and Dental Form
Two forms are required for admittance into Georgia schools: The first is the Certificate
of Immunization Form 3231, revised 1/2000 – According to current state law; we CANNOT
permit a child to begin classes at Providence until we have received this form.
The form may be obtained from your family physician or local health department.
The second form required for admission is the Vision, Hearing, and Dental Examination
Form Number 3300. This, too, may be obtained from your family physician or local
health department.
Step 4: Pastor’s Questionnaire
Please forward the questionnaire to a pastor, elder, or deacon to fill out. It should
be returned to us at their earliest convenience. The application process cannot
go forward until this form has been completed and returned to us. Print and download
the
Pastor Questionnaire Form, along with the
Providence Statement Of Faith to take to your pastor.
After All Forms Have Been Received . . .
Step 5: Testing
All applicants for kindergarten and first grade are required to take an admissions
test. Students applying for grades 2-12 may or may not require testing. Report cards
and the results of a nationally standardized test taken within the last 12 months
will be reviewed and considered. If no such testing records are available, or if
additional testing is necessary, the applicant will be notified. The basic testing
fee is $60.00 per student, and is payable at the time of testing. Additional testing
may be necessary at a fee not to exceed $150.00. Testing dates are scheduled for
January 10, 31 and February 21. Additional dates will be set as necessary.
Step 6: Interview
When all paperwork and test results have been completed, assuming there is space
available in the grade for which you have applied, and that the applicant is academically
and developmentally prepared, an interview will be set up between the applicant's
parent(s) and the Elementary and/or High School Principal. All sixth through twelfth
grade applicants will be interviewed during this time.
Step 7: Acceptance
Acceptance of the applicant will be based on the overall combination of test results,
transcripts, teacher recommendations, pastor questionnaire, and interview. In addition,
one of the unique things about Providence is that we are a “covenant school.” Our
mission statement indicates that we exist “to provide excellence in Christ-centered
education as an extension of the Christian home.” Consequently, at least one parent
in a family must profess a personal faith in Jesus Christ in order for their children
to be considered for enrollment. One hundred percent of our families at Providence
Christian Academy are active members of local churches. Important issues like this
one are discussed during a family’s personal interview with an administrator prior
to their student(s) being accepted at Providence.
Step 8: Registration
The registration form and acceptance letter will be mailed to the applicant upon
acceptance. The registration fee is required to hold a place for the student. A
student is not officially registered at Providence, until the registration forms
and fee are received.
